By proceeding further and clicking on the “Click to Request Medicines” link, I acknowledge and agree:
 
a.    that FTC Talent Media & Entertainment Private Limited (“FTC”) may collect, store, process, share my information/ data including personally identifiable information such as my or any other patient’s/ person’s name, contact number, email, copies of ID and address proof, medical details (either collected vide my use of FTC’s platform or this google form/application/ submission) with third parties (including BDR Pharmaceuticals Internationals Private Limited (“BDR”)) and I hereby consent to the same.
 
b.    that submission of this google form/application/ submission by me does not guarantee that (a) FTC shall provide my information/ data to BDR or (b) BDR will contact me or provide any products, services or medical advice/ assistance of any nature to me. I acknowledge that exercise of (a) and (b) mentioned herein above shall be at the sole and absolute discretion of these concerned entities.
 
c.    I shall not raise any claims against FTC on account of me providing the information/ data or BDR providing any products, services or medical advice/ assistance to me.
 
d.    Notwithstanding anything contained in this google form/ application/ submission, I shall not hold FTC liable (i) in case BDR does not contact me or does not provide any products, services or medical advice/ assistance to me or the patient for whom such google form/application/ submission is made by me; or (ii) for any claims, costs, liabilities, expenses, damages, losses arising out of or in connection with any products, services or medical advice/ assistance provided by BDR or any use or misuse or loss thereof.
 
e.    that any information on FTC’s platform in relation to this google form/ application/ submission is not intended or implied to be an undertaking, representation, warranty, guarantee, in respect of BDR or for any products, services or medical advice/ assistance provided by BDR; and that FTC is organizing this program on a good faith basis, without any consideration to endeavor to provide benefit to its users or customers or community during the pandemic on purely humanitarian grounds and as part of its corporate social responsibility.
 
f.    that FTC is only acting as a connecting link between its users or customers through the platforms (at www.ftctalent.com (Website) and the mobile application (App)) and this  google form/ application/ submission and that there is no joint venture, partnership or any relationship/arrangement of a similar nature with BDR; and hence FTC is not liable or responsible for any of the products, services or medical advice/ assistance provided by BDR or inaction on the part of BDR in relation thereto.
 
g.    that all content, including text, graphics, images and information, contained on or available through FTC’s platform or marketing or publicity material in relation to this form/ application/ submission or any action for this campaign, project or any content used in relation thereto are for general information purposes only.
 
h.    that all information/ data provided by me vide this form/ application/ submission is correct and not misleading. I shall indemnify and hold FTC harmless in case any loss is caused to FTC due to me having provided incorrect/ misleading information/ data.
 
i.    I wish to receive the products, services or medical advice/ assistance of BDR of my own accord and I have therefore provided my information/ data through this google form/ application/ submission. I further acknowledge that there has been no solicitation, invitation or inducement of any sort whatsoever from FTC or any of its representatives to provide such information/ data.
 
j.    I will not hold FTC and/or BDR liable for any losses, liabilities, costs and expenses of any nature and for any reason whatsoever.
 
k.    That I have read and understood this Disclaimer in its entirety and agree to abide by the same at all times.
